Matt Webb is moving to New Zealand  in Dec.  Perhaps Blake Cameron could take over as CE chair?

Eventbrite was a problem for one of the classes (May 2017 Pros--Mastership). A non-AGD person couldn't sign up.  Settings need to specify that non-members can attend.

Dave M is at capacity with his commitment at Roseman.  Is on the hook for next 2 CE courses but feels like he can't head the CE committee.  Has asked Duane and Ruedi to help at Roseman.  Aaron Ferguson suggested that Dave talk to Harmon (over clubs) at Roseman to ensure that the AGD is a legitimate club at Roseman.  He requested bylaws to submit to the administrative council to be recognized as a club prior to organizing the chapter.

March 2 course at Roseman: (research and evidence-based dentistry).  Frank Licari, Joe Cheever, Dave Prince, Kamran Awan.  No cost for the speakers to the AGD.   All profs at Roseman. split proceeds with Roseman student chapter.


Rose man chapter to be a sub account to the UAGD.  Chapter is a part of our constituency.  Budget is together.

Aug 31, 2016 Board Meeting

Finances--Lynn-the uagd is a 501c. scott got a letter saying we have an exception from the irs to be tax exempt. Otherwise, we have to prove to the irs we are exempt.  we can't advertise for a profit group to keep our exempt status.  there were papers filed.  we must send a 990 to irs each year explaining income and what is used for to not require a determination letter to continue.  we are included in the national AGD for this umbrella. 26K income 2015. expenses were 22K in 2015.
 Noone on board has looked into other investments other than CDs in past. we have 4 accounts--general, mastership and 2 others.  each account has a low interest savings bond. we'll discuss this again at the next meeting on sept 16.

Spring Regional Meeting-paul. April 28, 2016 5pm after the UDA convention. Region will sponsor. Duane will make arrangements for a room.

Board Positions--was recommended to have 2 year terms. Decided to keep a one year term for now.

Openings on the board-Dan Boston resigned as Mastership program advisor. VP (PresElect), Sec, We need to move a few people around.  Andy pres for next year. Scott will look within the board to fill the presidency and new recruits can fill in the committee vacancies.

Health insurance proposal--scott stucki wanted to explore ways to apply for group health care and use the larger uagd group. there are 5 questions he needs answered to determine interest. scott would like everyone to at least answer the questions in the newsletter to the contact there.

Newsletter- Paul --always recruiting for articles and information of any kind.

Jan 15, 2015 Board Meeting

Scott T to update leadership page with AGD.

HOD meeting- San Francisco this year.  not sure if nat'l will pay for delegate. Concerns for Advocacy should be addressed to Duane in newsletter.
JC would like to find someone to take over as RD.  Anyone interested should talk with him.  Dave McMillan may be interested.  Duane is willing to help out where possible.

Dave Peterson-Detroit meeting HOD was decided to separate Governance and Science meeting in 2019 and hold the HOD in Chicago at HQ.
Mastership-26 attendees. very successful.  November and May mtgs.  May 8-9.   Ortho/Pedo/sleep med. $400/.  Nov 6-7 special needs/geriatrics.  Aaron says Roseman is available so long as a room can be freed for the course.  The U is also available as is Ultradent.
Grant commented on the reviews for the November meeting.  Most were pleased.  2 had philosophical differences with Dr. Chan.   Roseman's graduation is June 6, 2015.
Matt has CE courses tentatively planned through May 2019.  It was agreed to hold off the change until 2016.  We discussed having CE in March and Sept.  Mastership will be held in May and November.
ADEA is held in March for the dental educators.

UDA Luncheon-JSMB Friday. No speaker yet.  Roseman has 300 students.  in the past, we have opened up the luncheon to the students for free.  This year, however, none of the students have class on that Friday.  we discussed a free meal for AGD members (student or not) and $30 for non-AGD.  The U students haven't organized a constituency yet and we haven't visited them either to stimulate interest.  Discussed offering the meal to the U students like we did for Roseman students last year.  All attendees must RSVP to Ruedi or Dan.  Lynn suggested a $10 fee for all to commit so we don't pay for empty plates.  Cap the attendees if necessary.
Agreed to look into a reasonable fee speaker, $10 fee, 1 week RSVP to Reudi or Dan
some speakers might cost some money -Alan Hall, Thurl Bailey, Eric Rirey (Biofire CEO) were suggested.  Dave McMillan saw their bio's on Linked In.

Matt Webb-CE  Mike Bennett will help org the sleep med for Sept CE course.
Dan Boston has given suggestions for the May Fellowship Review course.  First Friday in March and Sept beginning in 2016. Same dates for 2015.

Current board --need a new secretary. Other positions have moved up. Mike Bennett on both master and CE committees.

Board meeting 9-9-14

nominations for dentist of the year--Grant Quayle to be presented on the 19th.

PK Clark bone grafting course same day as AGD November 6-8 mastership course.  He has asked us to advertise the course and accept a 200 kickback per AGD member signing up.  We can let him advertise to our members.  Scott T has a concern about using the database this way.  Dr. Stucki to follow up.

400 for the UAGD mastership course.  Courtyard Marriott. fri and sat 8 hrs per day.  H night presentations 6:30-9:30pm. speakers arranged.  lecture 7-9 fri night @ ultradent by Clayton Chan open to anyone for $50. AGD to spend 2500. one sponsor at 1000 contribution. Dr. Chan's honorarium is 10K.  We need 100 people to pay the whole honorar otherwise he'll take what he gets  .  Neuromuscular occlusion.  Bring the team.  Fri Jim Guinn will speak 1/2 day hands on.  A flier is now ready to mail out for Fri night.  myotronics neuromuscular bite.  need casts. articulator. The whole weekend flier only will be sent out to certain people.  It's not ready yet.  David Peterson will get the info to Dave McMillan this week to send thru Alphagraphics.  There is only room for 34 at the  Master track course.  There was discussion about who to invite to what.

Our courses should be scheduled out at least a year in advance.  Just a few weeks in advance is not enough time--especially when we are planning multi-thousand dollar events.  When events are on the calendar and in the newsletter for several months, we will have better publicity, better attendance, more vitality and a better presence.

Scott Tracy is the Pres Elect and will set up a way to invite the U students to AGD events and be a student chapter.

The national meeting-- David Peterson was the Delegate.  2017 will be the last combined governance and scientific meeting.  JC wants to step down as the RD and have a replacement.

-Laser course well attended and successful.  Homework assignment is do mini thesis on your choice of case with laser.  Course certificates to be mailed to participants.
-September course.  Topic?  Will have AGD meeting during lunch.  Faculty and students at Roseman will attend course in limited numbers.
-November course on occlusion.  Fee to be $350  Multiple speakers to lecture at course.
-Advertise both courses at same time in UDA magazine and also e-mail blasts
-Donated Dental Services requesting list of all Fellows and Masters in Utah to send letter about Donated Dental Services
-AGD membership drop list.  40 dentists on list.  Plan to send letter from AGD pres. to reach out and help them resign with AGD
-Next teleconference or meeting in first or second week of August.

Board Meeting 4-17-14

Roseman Visit Evaluation. Many students joined. Aaron created power point presentation. Went very well. Could be a venue for a future Mastership course. Dr. Ferguson offered the venue.  Good Sim lab, nice facility. Lunch room on third floor on west.

UDA Convention: Luncheon was best attended in years. Many of Nathan's class attended. The students really appreciated the information on practice transitions.  Scott Tracy was wondering about organizing a committee for the luncheon.  Grant thought it would be a good idea too.  Involving students that will get time off could be a very good thing.  They are willing and we could communicate better with them.

Laser course: 30 signed up with 1 on the waiting list.  We need someone to pickup Dr. Convissar on Thurs night and return him on Saturday right after the course. Matt needs a CE certificate and report to AGD.  National just wants a plain document. David asked about a roster for the Mastership program. Grant to send certificate template from AGD website to Matt and David.  Course to finish at 1. flight leaves at 3 or 3:30. Paul to take him back to airport. Dave to pickup Thurs night at 7pm or ask Andy or David. Staying at Hotel next to Ultradent (1/2 mile south).  Matt to take from Hotel to Ultradent.  Participants to bring gloves, loupes and suture kit. someone to spot lunch with a CC and reimburse from course fees.

September annual meeting: Matt has discussed a date with Aaron at Roseman but will need to rearrange schedule to put people in Nursing area. 3 or 4 local specialists to give a pearls day.

November Mastership course:  6-8 of November reserved.  presentations on 6th (Thurs). Jim Guinn can't do 2 days so another speaker on Occlusion would be good.  5000 budget per course is variable.  wanted a full day for presentations, but the fewer presentations could be condensed into 2 days.  The criteria from national is specific.  Each person is to present for 15-20 minutes.  would need 16 hours, twice per year plus a homework assignment. 30 hours total per course.  need to get a group of people that are committed to doing the mastership.  arranged at Ultradent for next 5 years.  Dave proposed Mike Affleck to speak for 1 day.  Clayton Chan is a neuromuscular approach and puts on a course in NV called "Occlusal Connections".  He would do well.  Dr. Durham at the VA is a great lecturer.  A young prosthodontist.  teaches a 2 visit denture. knows materials well. may not lecturer on occlusion. Clark Dana does Occlusion at Roseman.

Donated Dental Services:  email from Dave
Hello Dr. McMillan,

Thanks for speaking with me on Friday about our interest in exploring possible collaboration with the AGD as we plan for a Donated Dental Services (DDS) program in Utah.  I apologize for the delay in sending this follow-up communication.

We appreciate your kind and gracious offer to share the information with your board during its March 14th meeting.  We would be honored and most grateful if the AGD would approve having a representative on an advisory committee we hope to establish, and depending on recommendations the group may suggest, consider if additional involvement of the Utah AGD would be appropriate.

I am copying Dr. David Peterson as I understand he serves on your board.  He is also a uniquely caring and generous DDS program volunteer, having been involved with the established project in Wyoming as well as the small Utah component of the DDS “safety-net.”  That is an initiative established to assist a limited number of needy disabled, elderly, and medically-compromised people in the 8 states where DDS programs have yet to be implemented.

God forbid that anyone, anywhere, is unable to obtain medically-essential but unaffordable and uncovered dental care if there is an accessible dentist willing to donate the treatment.  Somewhat less extreme, but equally compelling, it is tragic if someone who is financially challenged by functional limitations from disease, disability, and advanced age must live with the degradation of physical and emotional distress from serious dental problems if there is a nearby dentist who would value the opportunity to contribute needed treatment.  For radically different reasons, patients and dentists enjoy greater quality of life, and meaning, when brought together.  DDS is an extraordinarily simple and straightforward program to achieve that purpose.

DDS began as a small pilot project in Colorado during the late ‘80’s.  It has subsequently expanded nationally.  With the assistance of the AGD, ADA, and ten other national dental organizations that have endorsed DDS, 15,200 caring dentists volunteer along with 3,300 supportive laboratories.  More than 120,000 needy disabled, elderly, and medically compromised individuals have obtained $250 million of comprehensive dental therapies.  The following is a link to a recent ADA News article, http://www.ada.org/news/9522.aspx.  Social work coordinators staff the program.  Among their most consequential responsibilities is endeavoring to assure that volunteers and their staffs enjoy gratifying experiences free of annoyances.

Several program features are intended to foster mutuality of benefits for volunteers and patients.
·         Each volunteer controls his / her involvement.
·         Volunteers use their respective offices rather than working in unfamiliar settings.
·         Hundreds of private offices distributed throughout a state increase options for geographic accessibility among patients.
·         Volunteers are mailed a brief written profile of an applicant and requested to authorize a referral before one is made (first attachment).
·         Only after completing a clinical examination is a volunteer asked if s/he wants to treat the person.  If the dentist declines, the profile is sent to another volunteer.  Should the dentist agree to treat, as the overwhelming number do, the coordinator asks what assistance may be helpful, including arranging the involvement of volunteer specialists and labs, free replacement of zirconia blocks, the donation of implants and components as requested, and/or a Sonicare toothbrush to assist a patient maintain the contributed treatment.
·         Volunteers are fully supported should they want to provide comprehensive treatment for individuals with extensive needs.  Patients are thereby positioned to leave the program with excellent oral health, which they are expected to maintain independent of the program so volunteers can assist others if they choose to do so.

Dr. Petersen likes working with them in WY because they screen a pt's income and need and then pay lab expenses so the DDS only donates the work and patients are screened and takes the burden off the Dr. Dave to have Larry send Paul info for the newsletter.

Group insurance to AGD members group health insurance.  Joe Mirci was talking with Select Health IHC to look into a plan for dentists (and employees???).  May not have open enrollment till next year with ACA.

Affliation agreement with National and our constituent.  want the document to be shared tonight.
